Credentialed Mental Health Nurse Service

Our Credentialed Mental Health Nursing Service provides clinical and recovery-focused support for individuals living with severe and complex mental illness. Delivered by nurses credentialed through the Australian College of Mental Health Nurses (ACMHN), our services are tailored to promote independence and well-being.

Ideal candidates

  • Individuals aged 15+ with complex mental illness

  • Those at risk of hospitalisation or requiring extended support

  • Patients needing care coordination alongside existing services

Services include

  • Regular mental health and recovery reviews

  • Care coordination with clinical and community supports

  • Active participation in care planning

  • Strengths-Based Therapy, mindfulness, and motivational interviewing

  • Medication management and monitoring

  • Optional family/carer involvement

  • Physical health screening and metabolic risk assessments

  • Education on general health and wellbeing

Accessing Mental Health Nurse Support
