DVA Community Nursing Services

At Health On Call, we proudly deliver DVA-funded Community Nursing Services in partnership with the Department of Veterans’ Affairs. Designed for eligible veterans and war widows/widowers (holding a Veteran Gold Card, or a Veteran White Card for warranted conditions), our program provides expert nursing care at no cost to the patient.

Who’s eligible?

  • Gold Card holders: Full access to clinically-based nursing and personal care services

  • White Card holders: Funded services only if care needs stem from accepted service-related conditions

Our DVA Services include

  1. Wound and skin integrity care

  2. Medication administration and management

  3. Catheter and continence support

  4. Diabetes and chronic illness management

  5. IV therapy and tube feeding

  6. Ostomy and nutritional care

  7. Palliative care and end-of-life support

  8. Personal hygiene: bathing, dressing, grooming

  9. Coordinated post-hospital and transition care

Accessing DVA Services

1. Referral required from an authorised healthcare professional
2. Eligibility check for DVA card and care needs
3. Home assessment visit by our nurse
4. Service commencement as per DVA care plan
